The preliminary investigation report of the plane crash that took place on Tuesday, June 12, 2025 in Ahmedabad, Gujarat has been released.
The Aircraft Accident Investigation Bureau (AAIB) released a report late at night, which revealed the cause of the Air India international flight crash.
Air India Airlines today confirmed receipt of the preliminary investigation report on the plane crash on 12 July 2025 and also committed to fully cooperate with the AAIB in its further investigation.

The AAIB's preliminary investigation report attributed the accident to engine fuel failure. Data from the cockpit voice recorder showed that as the plane reached its maximum speed of 180 knots after takeoff, both engine fuel switches went from running mode to cutoff mode. Both switches are cutoff at 1 second interval. This stopped the supply of fuel to the engines.
The report said that when the speed of the plane started to decrease, one pilot asked the other pilot that why did you cut off the oil supply switch? The pilot replied that he did not.
The pilot immediately switched on. One engine recovered thrust, but the other engine did not activate. The speed of the plane started to decrease and it began to descend, so the pilot gave a May Day message.
The AAIB's preliminary investigation report stated that the emergency was declared by ATC as soon as the Mayday call was received. Photography and videography of the accident site was done using drones. The wreckage of the crashed plane was examined in a forensic lab. The wreckage has been kept safe at Ahmedabad airport itself. Both engines of the plane were recovered from the wreckage, which is kept separately in the hangar at the airport.
The report said that evidence has been identified and gathered for further investigation of the plane crash. Testing of fuel samples taken from bowers and tanks used for refueling planes was done at the DGCA lab. Very small amounts of fuel were sampled from the APU filter and left wing refuel/jetson valve. Statements of witnesses and surviving passengers were also recorded.
What happened before the accident?
The AAIB, releasing its preliminary report a month after the tragic accident, said the aircraft's air/ground sensors went into air mode, consistent with take-off at 08:08:39 UTC.
"The aircraft attained a maximum recorded airspeed of 180 knots IAS at approximately 08:08:42 UTC and immediately thereafter the engine 1 and engine 2 fuel cutoff switches transitioned from run to cutoff position one after the other with a time interval of 01 seconds," the report said, citing the aircraft's Advanced Airborne Flight Recorder (EAFR). According to reports, engines N1 and N2 started to lose their take-off values due to fuel supply cut off.
"In the cockpit voice recording, one pilot is heard asking the other why he cut-off. The other pilot replied that he did not," the report said.
While detailing the accident, the AAIB said CCTV footage obtained from the airport showed the RAM Air Turbine (RAT) being deployed during the initial climb shortly after take-off.
"No significant bird activity was observed around the flight path. The aircraft started to lose altitude before crossing the perimeter wall of the airport," the report said. According to the EAFR, engine 1's fuel cut-off switch transitioned from CUTOFF to RUN at approximately 08:08:52 UTC. Subsequently at 08:08:56 UTC, engine 2's fuel cut-off switch also changed from CUTOFF to RUN.
The flight to London had crashed.
